To start, you can lay on your Traction Pillow anytime. Next, you can lay with your your head hanging off the bed. I usually spin around so my head hangs off where my feet usually are. Last, when I'm sitting in a chair with a high enough back, I let my head fall back in extension. I like it when the back of the chair is at the spot where I want the most traction.
Of note, is watch the amount you do, you can be sore if you do too much. Also, make sure the spot that hits your neck isn't too hard.

"I Sleep With Two Pillows!"
I've talked to some people who do, and I don't recommend it. One reason it often comes up is that they'll get the Traction Pillow and say it's too high. Then you find out they are putting it on top of another pillow.
It's not that they need the small Traction Pillow, you should just be using one pillow.
Two pillows pushes your head so forward, not to mention when you're on your side, it cranks the head to one side or another. The goal is to keep your spine in alignment - and pushing it in any direction doesn't do that...
